ODJFS papers and CPR/1st Aid Cards will only be issued at time of class for facilities in good standing-no unpaid/open invoices past 30 days. Payment options include: pay for class online, send payment with student, authorize 1st Choice CPR to use credit card on file OR we can continue to send invoices for classes.  Certifications will then be mailed with your receipt. 

NO personal checks.

Certifications will be given for offsite classes upon receipt of payment.

Cancellations for offsite classes must be made and confirmed at least 24 hours before scheduled class.  Cancellations less than 24 hours from class may be charged $50 per instructor that was scheduled to teach at the class.

Initial (2 Day) ACLS & PALS courses require a $50 non-refundable deposit.  The deposit will be applied to the cost of the class.
